Appointment information: Call to schedule an
appointment.
Information: Not-for-profit,
community-based/freestanding hospice; Medicare/Medicaid
certified; licensed as full hospice service by Illinois
Department of Public Health; contracts for hospital hospice
in-patient care and nursing home care.
Services offered: Interdisciplinary program of care that
offers symptom management, psycho/social and spiritual care
for terminally ill patients and their families. Care
offered in patient’s home-like setting; includes regularly
scheduled R.N. (nursing), home health aides, social worker,
and/or pastoral care. Trained volunteers available for
patient/family visits and support. Bereavement counseling
for families up to 15 months after patient’s death.
Children’s grief program (Heartlinks).
Persons served: Terminally ill patients of all ages
and their families. Heartlinks is open to community.
Fees: None. Costs covered by Medicare, Medicaid and
most insurances. Heartlinks funded by community programs
and grants.

requested subjects are available around the clock.
Services offered: Serves as a one-stop clearinghouse
fro inquiries about federal and state government functions.
Provides answers to questions about the government or
government-funded programs or refers inquiries to the
appropriate office.
Persons served: All.
Fees: None.

Services offered: The meetings are informal. If
there is not a guest speaker an open discussion of “Care &
Share” is held. New attendees receive a take home packet.
A meeting reminder is sent to everyone on the meeting list.
Persons served: Anyone who has fibromyalgia, families
and friends are welcome.
