institutionalization of individuals. ADDUS Healthcare is a
service provider contracting through the Community Care
Program sponsored by Illinois Department on Aging. Services

services. No age limitations.

Youth Leadership Development and Community Empowerment.
Services offered:
Leadership Institute Program for high school students 9th
– 12th grade who have demonstrated leadership
potential and scholar merit. Leadership Lecture Series
Program community leaders speaking to high school and
college students. Leadership Recreational Program
stresses sportsmanship and physical development for 5-14
years old. Summer Leadership Program is an
educational and recreational program for K-9th
grades. Youth Forum Programs on various subjects.
PAVE Program (Parents Are Vital in Education) is
composed of neighborhood parents whose children attend one
of the after school SAFE HAVEN CENTERS. A Rite of
Passage program specifically focused on leadership
development and adulthood training for African American
fifth grade students. A Sister to Sister-Brother to
Brother mentoring program where mentors create a
positive, willing and enabling environment for 3rd
grade students. A Family Centered Service program
that provides intervention services to children (and their
parents) who suffer with behavioral problems.
